Am I A Candidate?
If you can answer "yes" to these questions, you may be a
candidate for bariatric surgery:
Have you
been dieting for more than five years?
Have you exhausted
all other means of weight loss?
Are you between
the ages of 18-60 years old?
Do you have
a BMI of 40?
Do you have
a BMI of 35 and an obesity related illness? (diabetes,
hypertension, hypercholesterolemia or sleep apnea?)
Are you ready
to make a significant change in your life?

PLEASE NOTE
Patients who live more than 45 minutes drive from Maimonides
Medical Center must stay within 45 minutes drive of the
hospital for 7-10 days after discharge.

Have your insurance information available. The staff will
ask a few basic questions regarding your height, weight and
health background. You will need to fill out the enrollment
packet as completely as possible and bring it with you to your
first meeting. You may request to have one mailed to you, or
print
out a copy on line. |

Please discuss your interest
in surgery with your primary care doctor. It is mandatory
that all patients have a primary care physician who
will work with them after surgery. The support of your
primary care physician may be necessary when working
with your insurance carrier.

INSURANCE COVERAGE?
You need to make yourself aware of the requirements
of your insurance policy.
Please educate yourself about your insurance coverage:
- Do you require referrals to see a specialist?
- What is your co-payment?
- Do both surgeons and the hospital participate
in your specific insurance plan?
- If not, do you have out of network coverage?
- Are there any specific exclusions regarding
the surgical treatment of "morbid obesity"
in your contract?

INITIAL INTAKE APPOINTMENT - WHAT TO BRING
- Primary care physician's name, number and address
- List of current medications
- Insurance card
- Referral (if necessary, your insurance requirements
are your responsibility)
- List of questions
- Information from your cardiologist, neurologist
or pulmonologist, if applicable
- If you speak a language other than English, you
MUST bring someone who is capable of translation.
- Please do not bring children; you must be able to
focus on the information presented.
- We recommend you bring an adult family member.
